A absorção do ferro não heme através da membrana apical do enterócito é mediada por um transportador
identificado como transportador de cátion ou metal divalente (Nramp2/DCT1/DMT-1)

O segundo mecanismo é baseado no nível de ferro dos estoques do organismo, (regulação pelos stoques). A regulação pelos estoques pode aumentar a absorção de ferro em duas ou três vezes em condições de deficiência de ferro. Aparentemente, a absorção do ferro disponível na dieta é indiretamente influenciada pela saturação da transferrina no plasma.

Frazer & Anderson propuseram que a hepcidina (Liver-Expressed Antimicrobial Peptide (Human)LEAP-1/ Hepcidina), um peptídeo de 20 a 25 resíduos de aminoácidos, isolado inicialmente da urina e sangue humano, mas com expressão predominante no fígado e dependente da ligação de ferro aos IRE, seja o sinal solúvel
que modula a expressão dos transportadores de ferro nos enterócitos. A hepcidina interage com o transportador de ferro da membrana basolateral dos enterócitos da cripta intestinal, promovendo eventos intracelulares que regulam a expressão do transportador de ferro (DMT-1) na membrana dos enterócitos, dependendo do grau de deficiência de ferro.
A homeostase do ferro seria regulada pela expressão de hepcidina no fígado, em resposta a mudanças na quantidade de transferrina diférrica circulante, detectada pelo receptor para transferrina 1 e 2 e pelo complexo HFE/receptor para transferrina. A deficiência de hepcidina causa sobrecarga de ferro enquanto que a superexpressão causa deficiência de ferro.

Hepcidin is a peptide hormone that regulates iron homeostasis and acts as an
antimicrobial peptide. It is expressed and secreted by a variety of cell types
inresponse to iron loading and inflammation. Hepcidin mediates iron homeostasis
by binding to the iron exporter ferroportin, inducing its internalization and
degradation via activation of the protein kinase Jak2 and the subsequent
phosphorylation of ferroportin.

Hepcidin is synthesized in hepatocytes, secreted from hepatocytes, and excreted through the kidneys.(43,47) Hepcidin in its mature form is a 25-amino acid-peptide, hepcidin-25.(47) Hepcidin-25 binds with ferroportin and both are broken down in lysosomes by endocytosis.(46,48) Hepcidin-25 expression decreases and ferroportin iron export is promoted, when the body requires more serum iron, such as iron deficiency, hypoxia, and pregnancy.(47,4950) However, in iron deficiency anemia in states of chronic inflammation, the supply of iron to the blood is inhibited by the reduced ferroportin function. It is thought that the inflammatory cytokine, interleukin-6 (IL-6), induces the expression of hepcidin-25 in hepatocytes, resulting in decreased ferroportin.(49,51) And when serum iron is not needed, hepcidin-25 expression increases and ferroportin is broken down, restricting the supply of iron.(47,49)

USO DE ALBUMINA E RISCO DE MORTALIDADE

Use of albumin as a risk factor for hospital mortality among burn patients in
Brazil: non-concurrent cohort study.

CONTEXT AND OBJECTIVE: among burn patients, it is common to use colloidal
substances under the justification that it is necessary to correct the oncotic
pressure of the plasma, thereby reducing the edema in the burnt area and the
hypotension. The aim here was to assess the risk of hospital mortality, comparing
the use of albumin and crystalloid solutions for these patients.
DESIGN AND SETTING: non-concurrent historical cohort study at Faculdade de
Medicina de Marília; within the Postgraduate program on Internal and Therapeutic
Medicine, Universidade Federal de São Paulo; and at the Brazilian Cochrane
Center.
METHODS: burn patients hospitalized between 2000 and 2001, with registration in
the Hospital Information System, who received albumin, were compared with those
who received other types of volume replacement. The primary outcome was the
hospital mortality rate. The data were collected from files within the Datasus
software.
RESULTS: 39,684 patients were included: 24,116 patients with moderate burns and
15,566 patients with major burns. Among the men treated with albumin, the odds
ratio for the risk of death was 20.58 (95% confidence interval, CI: 11.28-37.54)
for moderate burns and 6.24 (CI 5.22-7.45) for major burns. Among the women, this
risk was 40.97 for moderate burns (CI 21.71-77.30) and 7.35 for major burns (CI
5.99-9.01). The strength of the association between the use of albumin and the
risk of death was maintained for the other characteristics studied, with
statistical significance.
CONCLUSION: the use of albumin among patients with moderate and major burns was
associated with considerably increased mortality.
